Skilled Nursing Care

A skilled registered nurse will come to your home within 48 hours of receipt of a referral to determine the immediate care and support needed. Nursing services include but are not limited to:

• First visit within 48 hours of referral by a physician to determine immediate care and support needs of you or your loved one,
• A comprehensive plan of care
• Thorough initial and ongoing assessments
• Medication administration
• Treatments, diagnostic and preventative procedures
• Teaching (including medication teaching, nutritional guidance and dietary teaching)
• Supervising and counselling of you and your family regarding your nursing needs
• Coordination of  your total care notifying the physician and other members of the care team of your progress and needs
