Our home care services team fosters relationships and promotes independence, with caregivers who are flexible, supportive, and passionate about improving lives for your loved one.
A skilled registered nurse will come to your home within 48 hours of receipt of a referral to determine the immediate care and support needed. Caregiving services include but are not limited to:
- First visit within 48 hours of referral by a physician to determine immediate care and support needs of you or your loved one,
- A comprehensive plan of care
- Thorough initial and ongoing assessments
- Medication administration
- Treatments, diagnostic and preventative procedures
- Teaching (including medication teaching, nutritional guidance and dietary teaching)
- Supervising and counselling of you and your family regarding your nursing needs
- Coordination of your total care notifying the physician and other members of the care team of your progress and needs
